Welcome to the AT&T ETF Buyout Program for ADI. Please complete the fields below to start the process for getting your ADI customer their ETF rebate check. After you setup an account with a username and password, you will have access to the form to fill out your customer’s information so that we can request a bill from them with the ETF amount on it. You will be able to sign back in to check the status of the rebate check and to fill out additional forms.
